Are employers and their service agents in the Department of Transportation (DOT) drug and alcohol testing program required to obtain employee written authorizations in order to disclose drug and alcohol testing information?

· I am bound to protect each client’s information.
· However, there are limits when I must release information.
· I must release information to the appropriate authorities, if there is a possibility that you may hurt yourself or others.
· I must report suspected child abuse to the authorities and the Department of Family Services Child Protection Services.
· I must respond to court orders signed by a judge.
· I may ask you to sign Release of Information forms to assist in your care for medical professionals, DCPP, Probation, DOT/back-to-duty assessments, employers, and anyone else that you designate that may assist you in your treatment.
